Register Your Child Today

The °ÅÀÖÊÓƵ¹ÙÍøCentral School District now accepts fully online registration when enrolling your child in the school district.  Please select the type of Registration you wish to complete from the list below for specific information and documents required. If applicable, please be sure to select the proper school year you are completing the registration for.

If you are a returning student to the °ÅÀÖÊÓƵ¹ÙÍøCentral School District, please contact the Registration Office at registration@smithtown.k12.ny.us.

Universal Pre-Kindergarten: Applications for the 2025-2026 School Year are available through the Universal Pre-Kindergarten (UPK) section below and are being accepted through February 28, 2025.

Universal Pre-Kindergarten (UPK)

To apply for the Universal Pre-Kindergarten lottery for the 2025-2026 school year, please complete the Online Registration Form found .

°ÅÀÖÊÓƵ¹ÙÍøis offering full time Universal Pre-Kindergarten for the 2025-26 School Year.  Eligible students must be 4 years old on or before December 1, 2025, and born between December 2, 2020 and December 1, 2021.  Students may not be eligible for Kindergarten for the 2025-26 school year to enroll in the Pre-K program and must be residents of the °ÅÀÖÊÓƵ¹ÙÍøCentral School District.

Please note that spaces are limited and signing up for the lottery does not guarantee you a place in the Pre-K program.

Grades K-12 (2024-2025 School Year)

To register a child in grades K through 12 in one of the °ÅÀÖÊÓƵ¹ÙÍøCentral School District public schools for the 2024-2025 school year, please complete the Online Registration Form found .  Students entering Kindergarten in September 2024 must be 5 years old on or before December 1, 2024. Please note, prior to starting the registration process, please have electronic versions of the following documents ready to be uploaded:

  • Child’s Birth Certificate
  • Child's Immunization Records
  • Proof of Residency (ie-Lease Agreement, or Deed, or Two Current Home Utility Bills). Note: If you need to use two current home utility bills, please combine them to have a single electronic document for upload.
  • Parent/Guardian Photo ID. Note: Please cover the ID number on the photo ID.
Grades K-12 (2025-2026 School Year)

Please note: If your child is already registered for Special Services and/or UPK, kindly email registration@smithtown.k12.ny.us before continuing.

To apply for transportation and textbooks for a child enrolled in Private or Parochial school, please complete the Online Registration Form found .  Please note, prior to starting the registration process, please have electronic versions of the following documents ready to be uploaded:

  • Child’s Birth Certificate
  • Proof of Residency (ie-Lease Agreement, or Deed, or Two Current Home Utility Bills). Note: If you need to use two current home utility bills, please combine them to have a single electronic document for upload.

Special Services - Preschool

To register a child enrolled for Special Education Services or for a Special Education Evaluation, please complete the Online Registration Form found .  Please note, prior to starting the registration process, please have electronic versions of the following documents ready to be uploaded:

  • Child’s Birth Certificate
  • Child’s Immunization Record
  • Proof of Residency (ie-Lease Agreement, or Deed, or Two Current Home Utility Bills). Note: If you need to use two current home utility bills, please combine them to have a single electronic document for upload.

Special Services - Private & Parochial

To register a child enrolled in a Private or Parochial School for Special Education Services or for a Special Education Evaluation while attending a Private and Parochial School, please complete the Online Registration Form found .  Please note, prior to starting the registration process, please have electronic versions of the following documents ready to be uploaded:

  • Child’s Birth Certificate
  • Child’s Immunization Record
  • Proof of Residency (ie-Lease Agreement, or Deed, or Two Current Home Utility Bills). Note: If you need to use two current home utility bills, please combine them to have a single electronic document for upload.
  • Parent/Guardian Photo ID. Note: Please cover the ID number on the photo ID.

 

Registration Process

5 Easy Steps to Register Your Child

1. Proof of your child's age.

Ways to show a child's age:

  • Birth Certificate - The birth certificate has to be an original or a certified copy (which would come from the town/state/country the child was born in).   The certified copy would have an official seal (most likely raised).
  • Baptismal record (from any country)
  • A Passport (from any country)

If you don't have a Birth Certificate, baptismal record, or passport, you can use other documents if you've had them for at least two years, such as:

  • Driver's license  Note: Please cover the ID number on the photo ID.
  • State or governmental ID
  • School photo ID with date of birth
  • Consulate ID card
  • Hospital or health records
  • Military dependent ID card
  • Other documents from federal/state/local agencies (examples: Department of Social Services, Office of Refugee Resettlement)
  • Court Orders
  • Native American tribal document
  • Records from international aid agencies or voluntary agencies
2. Your own Photo ID.

Your own Photo ID such as a driver's license or passport.  Note: Please cover the ID number on the photo ID.

3. A record from physician with all inoculations listed with a doctor's signature or stamp.

4. Proof of Residency.

Parents/guardians must provide sufficient proof that they reside in the School District when enrolling their children in School District schools.  Examples of proof of residence include, but are not limited to:

  • A mortgage statement or deed of ownership;
  • A tax bill for the Town of Smithtown;
  • A copy of a lease executed by the tenant and landlord, with an affidavit by the landlord that the lease is in effect with a copy of the landlord’s mortgage statement or deed of ownership;
  • Statements for utility bills;
  • The portion of a current telephone bill showing name and address;
  • A moving bill from a commercial moving company;
  • A statement from an attorney stating that he certifies that the individual resides at a specific address within the °ÅÀÖÊÓƵ¹ÙÍøCentral School District;
  • A pay stub or income tax form showing the in-district address;
  • A copy of each: a driver's license and insurance identification card;
  • A voter registration document or a state- or other government- issued ID;
  • Documents issued by federal, state or local agencies (e.g. local social service Agency, federal Office of Refugee Resettlement); or
  • Membership documents based upon residency.
5. Proof of custody or guardianship.
  • or an affidavit (Parent or Guardian) saying that you have "total and permanent custody and control" over the child
  • or other proof such as documentation that the child has been placed with a sponsor by a federal agency
