Health Office

All students who require medications during school hours must have a medication form filled out by their physician and signed by the parent/guardian. All medication (even over the counter medication such as Tylenol or Advil) must be brought into the Health Office by the parent/guardian in the original container with the student’s name clearly labeled on the container. Please contact the Registered Nurse in your child’s building if you have any questions.

As of July 1, 2018, New York State is changing the mandated health examination requirements. Physicals are now required for kindergarten and grades 1, 3, 5, 7, 9 and 11.  Physicals must be submitted/mailed to the nurses’ office within the first 30 days of school.  Proof of doctor appointment for mandated physical years will be accepted within the 30 day period.

If your child’s physical is less than a year old as of the first day of school, you do not need another physical. Any further questions, contact (631) 382-2104.  The office is open during the summer months from 8:00 am – 3:00 pm.

Students new to °ÅÀÖÊÓƵ¹ÙÍøSchools

All students new to °ÅÀÖÊÓƵ¹ÙÍøSchools need to provide an Immunization Record from their doctor in order to register for school and provide the school they are attending with a recent physical.  If no physical is provided the school doctor will perform a physical in the Health Office.   All new students will receive vision, hearing and scoliosis screening according to NYS guidelines. 

Elementary Students

All students entering Kindergarten, grade 1, grade 3 and  grade 5 are required to submit a recent physical to the Health Office.  If no physical is handed in the student will receive a physical exam by a school doctor. All students will receive vision, hearing and scoliosis screenings as per NYS guidelines. All physical education excuses and absence notes go to the Health/Attendance Office.  Any student who requires the use of crutches or Special Transportation needs their doctor’s note to state the accommodation and a time frame the student will need the accommodation (i.e. two weeks).  

Middle School Students

All students entering grade 6 must provide proof of varicella (chicken pox) disease or immunization from their physician. Sixth grade students also must provide proof of TDAP vaccine by the time they reach their 11th birthday. All students in grade 7 MUST submit a physical exam to the Health Office or the school doctor will perform the exam in the Health Office.   Students who wish to participate in any sport MUST have a physical by their doctor and it must be documented on the school district forms.   All students will receive hearing, vision and scoliosis screenings as per NYS guidelines. All physical education excuses and absence notes are to be turned into the Health/Attendance Office. Students requiring Special Transportation, crutches, 5 minute/elevator/escort MUST have the doctor’s note state so and include a time frame for any accommodation (i.e. for two weeks).

High School Students

All students entering  grade 9 and grade 11 must submit a physical exam to the Health Office or the school doctor will perform the exam in the Health Office. Students who wish to participate in sports MUST have a physical by their doctor and it must be documented on the school district forms. All students will receive hearing, vision and scoliosis screenings as per NYS guidelines. All physical education excuses must be turned into the Health Office. Can My Child Attend School Today?

Students with symptoms of respiratory or gastrointestinal infections, such as cough, fever, sore throat, vomiting or diarrhea should be sent home or directed to stay home. If at school, the individual should wear a well-fitted mask while arrangements are made to go home even if they have not yet had a test.

Students with COVID-19 symptoms should remain at home. Testing is recommended for people with symptoms as soon as COVID-19 symptoms begin. Students that are COVID-19 positive must isolate for 5 days with Day 1 being the first full day after symptom onset or the day after receiving a positive test result if no symptoms exist.

Students that have tested positive for COVID-19 may return to school after 5 days if:

  • They are fever free without fever reducing medicine for 24 hours and their symptoms are improving
  • They wear a mask days 6-10

Students who choose not to test for COVID-19 must stay home and isolate for 5 days or provide a note from a medical provider indicating an alternative diagnosis.

COVID-19 Testing Information

Students and staff with symptoms of illness should not attend school. It is recommended that individuals and/or families consult their medical provider to determine if COVID-19 testing is warranted and for further guidance. Students and staff who test positive for COVID-19 should follow the established CDC/NYSDOH guidelines for ending isolation.

Staff members will be trained on symptom and illness identification and instructed to observe all members of the school community for signs of illness. Anyone identified with possible symptoms or illness will be directed to the Health Office (school nurse) for further evaluation. Any individual who screens positive for COVID-19 exposure or symptoms, or who presents with a temperature greater than 100.0°F, will be sent to the Health Office for further evaluation. In the event a school nurse is not available, building administrators will ensure that the child or staff member is sent home and directed to contact their healthcare provider for additional evaluation.
